A artéria vertebral pode ser vista em sua totalidade, com quatro segmentos: pré-vertebral (V1), cervical (V2), atlântico (V3) e intracraniano (V4). Os ramos mais importantes se originam do segmento intracraniano: ramos meníngeos (que irrigam as meninges da fossa posterior), artéria cerebelar posterior inferior (P.I.C.A.). A artéria vertebral termina se unindo à artéria vertebral contralateral para formar a artéria basilar. Alguns ramos da artéria basilar também devem ser reconhecidos: as artérias labiríntica e pontina podem ser observadas, bem como as artérias cerebelares anteriores inferiores (A.I.C.A.), as artérias cerebelares superiores e as artérias cerebrais posteriores, que são os seus ramos terminais.
